New Horizons Home Care provides certified nursing assistants and in-home aides to help clients at home with personal care and home management tasks. Our staff have been trained in the care of the elderly and disabled in the home environment and we are dedicated to serving the needs of our clientele. Some of the services we offer include bathing and grooming assistance, meal preparation, laundry and linen changes, light housekeeping and other chores such as mopping, dusting and sweeping, grocery shopping, pharmacy pickups, and assistance with the management of medications. We work with your physician and other healthcare providers to ensure that your best interests are considered and that you receive the kindest and most caring support available.

Client Services

In addition to personal care services for qualified persons and CAP services for children and disabled adults, New Horizons Home Care, Inc. provides a wide array of services to CAP-MR/DD recipients and their families. These services provide assistance to clients who have developmental delays and disabilities and who are participants in the CAP-MR/DD program. Our services include Personal Care Services, Respite and relief services for families, and Supported Living Services, which help the clients to work on daily goals in order to achieve the highest possible level of independence. Our agency is also privileged to provide CAP-MR/DD Case Management services to clients and their families. Our case managers work hard to coordinate with outside resources to help the client obtain necessary services in order for them to remain in their homes and communities.

Serving clients in Yadkin, Surry, Wilkes, Davie, Forsyth, Iredell, Rowan, Catawba, Cabarrus, Alleghany, Davidson and Alexander Counties and the surrounding area.
